Enrollment Form (open enrollment is in December of each year for a January renewal); new employees may enroll effective the first day of the month following 60 days from date of hire (for example, if you are hired on May 15th, enrollment becomes effective August 1st. You may enroll at any time if you experience a qualifying event. Contact the Budget Officer for more information.
